Hearing Aid Fitting Fee

$250 per Hearing Aid
|
| Fitting fee covers initial visit for
instruction and use of hearing aid.
Customized programming of your hearing aid.
ALL follow-up visits for programming and adjustment during the
first 2 months from your initial fit date. This fee also applies to
hearing aids purchased at other clinics or on the internet when a re-fit
is performed.
Hearing Aid Fitting Fee is for services rendered and is therefore
non-refundable.

Hearing Aid Cost
|

| Hearing aid pricing is based on model, size,
and technology. The cost of
the hearing aid includes the warranty given by the manufacturer.
Warranty time will vary depending on the hearing aid model (minimum
warranty 1 year, maximum warranty 3 years). Warranty covers repair
charges from the manufacturer. Warranty
does not cover office visits for programming changes after the initial 2
months. One
time Loss and Damage is included by the manufacturer for the duration of
the warranty period but is subject to a $250.00
replacement fee.
Satisfaction Gauranteed - If you are
unhappy with your hearing aids for any reason, you may return them to
Southwest Hearing Solutions, Inc. within 45 days of your fitting for a
full refund of the hearing aid cost.

Office Visit

| Office visits include consultation with the
audiologist to check your hearing aids, hearing aid programming changes
and adjustments, and hearing aid cleaning.

Clean and Check
|

| This service applies to hearing aids dropped
off for cleaning and evaluation where no consultation with the
audiologist is required. You
will be notified when your hearing aids are ready for pick-up and if any
malfunctions are identified that may require repair.

Tubing Change
|
$5 per Hearing Aid
|
| This charge is in addition to the clean and
check fee for BTE hearing aids requiring new tubes for earmolds.
This fee includes an extra set of tubing with each change for all
Open Ear fitting systems.

In-House Repairs
|

|
| In house repairs refers to replacement of
broken battery doors, shell repair, and ear mold modification (after 60
day earmold fit warranty expires). If we can fix it in the office we
will.

Hearing Aid Repairs Sent Out
|

| This fee covers all repairs that are sent in
the any of the manufacturers or all make repair centers. Fee includes
one year warranty on repairs only.
There is an additional charge
of $50.00 for any hearing aid sent for repair over 5 years old.

Hearing Aid Remake

| Fee includes ear mold impression taken by the
audiologist for remake of custom hearing aids with shell damaged when
out of warranty. Fee also
includes one year warranty for fit and repairs following service.

Ear Mold Impressions
|
$25 per Ear
|
| Fee is charged for ear mold impressions for
custom hearing aids, hearing protection, headphones etc… Fee is due
the day the impressions are taken.

Custom Ear Molds

| The cost of custom ear molds is based on
model, style, and type. You
pay the cost of the ear mold from the manufacturer plus a $5.00 fitting fee when you pick up the product.

Ear Wax Removal

| Comprehensive fee includes removal from one
or both ears. If removal is
attempted but can not be completed this charge is waived.

Home Visit
